ADULT MENTAL HEALTH UNIT

ST VINCENT'S HOSPITAL SITE

  • The Adult Mental Health Unit is the gatekeeper of the 5 statewide beds for Aboriginal patients in the St Vincents psychiatric ward
  • This involves attending the ward round meetings weekly
  • Regularly visiting patients
  • Involvement in discharge planning

HOME BASED OUTREACH

  • Home based Outreach supports the Adult Mental Health Unit.
  • The Aboriginal Health Workers support their clients by
  • Encouraging and supporting ongoing psychotherapeutic treatments and health checks
  • Maintaining and creating links between the client, family, community and mainstream services
  • Supporting the clients to develop personal skills to be able to manage living independently at home and within the community.

HOME BASED OUTREACH COMMUNITY HOUSES

  • This program has 4 x 2 bedroom units in the community
  • Each unit houses 1 client with the other bedroom available for family or carers to stay over
  • These clients can have a number of psych admissions
  • Home based outreach assists towards independent living skills
  • There is no real time limit a client can remain in the units for
